Today is International Safe Abortion Day. From @unitednationshumanrights: “The world has been experiencing promising developments but also important setbacks affecting the right to sexual and reproductive health, including on the right to a safe abortion. Human rights law stipulates that women must be free to decide when to be pregnant, how many children to have, and how to space pregnancies. The full enjoyment of sexual and reproductive health rights is indispensable to women’s and girls’ ability to exercise all other human rights and for the achievement of gender equality. Laws and policies that deny women and girls their sexual and reproductive health rights are inherently discriminatory. The setbacks experienced during the last years on ideological grounds, together with the effects of the COVID-19 pandemic, have had negative effects on those in vulnerable situations or those historically subjected to discrimination, in particular girls, Black and Afro-descent women, indigenous women, migrant, refugee and internally displaced women, women living in rural areas, women with disabilities, LBTI women and gender-diverse persons and communities, and women living in a persistent state of crisis, among others. According to the World Health Organization, between 14,000 and 39,000 maternal deaths per year are caused by the failure to provide safe abortion. If a decisive step is to be made towards the achievement of the UN Sustainable Development Goals on gender equality and health and well-being, in particular those aimed at ensuring universal access to sexual and reproductive health rights and reducing maternal mortality, it is of paramount importance to prevent unwanted pregnancies through access to contraception, safe abortion services and quality post-abortion care. Women and girls should not be instrumentalised to serve fundamentalist ideologies and populist agendas. Religion and culture should not be misused to discriminate against and further oppress women and girls in a global context of severe backlashes against gender equality. Such access should happen in regular as well as humanitarian settings.” #abortionishealthcare

🚨TRIGGER WARNING! This post discusses pregnancy loss (PL) at <20 weeks and subsequent management options. Resources: @acog_org and UpToDate. Please review our previous post on Induced abortion (IA). Many of the medical procedures and medications used to perform IA in the U.S. are also crucial for treating and managing PL [ie miscarriage, spontaneous abortion (SA)]. As a result, the legal restrictions to abortion also threaten to limit access to the procedures and medications used for treating PL and prevent medical staff from offering optimal treatment to patients experiencing PL. Bans on medications for IA could occur with the reversal of Roe vs Wade since 20+ states have introduced bills that would restrict or ban medications used for IA. These bans could impact the management of PL, given that two of the drugs (mifeprostone and misoprostol) used for IA are also used to manage PL. This is already occurring in states with abortion bans and restrictions with reports of pharmacists and pharmacies refusing to fill prescriptions for drugs prescribed for pregnancy loss or ectopic pregnancies. In addition, states with restrictive abortion laws are already having providers who are unwilling to do surgical procedures to treat PL. The inability of providers to offer medication management as an option to treat PL could result in more surgical procedures being done, which is problematic for numerous reasons, namely a failure to offer patients proper and appropriate care. There is a common misconception often repeated by anti-choice advocates that amplifies the narrative and occurrence of abortions later in the course of pregnancy- but this is far from the reality of abortion care in the US. Most abortions occur before 13 weeks of gestation. Only 1% after 21 weeks. Furthermore, such simplistic narratives completely neglect the nuance of each of these cases, and all the potential complications that may arise during pregnancy that force an individual to make such a difficult decision, especially after 21 weeks. Induced abortion is an essential part of healthcare. It should be accessible and safe. Bias in medical research often obfuscates good engineering design. This reel describes how pulse oximeters work and how they failed some minorities during the C-19 pandemic. Note: Because Beer and Lamberts law cannot be applied strictly, there would be errors if they were used to directly calculate oxygen saturation. A solution to this is to calibrate to correct for errors. In addition, in a body part such as a finger, skin and other tissues also absorb some light. This poses a problem , because the pulse oximeter should only analyse arterial blood while ignoring the absorbance of light by surrounding tissues. Luckily, arterial blood is the only thing pulsating in the finger. Everything else is not. Any “changing absorbance” must therefore be due to arterial blood.The pulse oximeter uses clever circuits to extract the “changing absorbance” signal from the total signal.
